The ECG shows regular P waves with terminal P-inversion in V1 (left atrial abnormality). There is no evidence of conduction (complete heart block). The QRS complexes are regular at a rate slightly less than 40 beats per minute with a duration of 0.16 sec and morphology consistent with right bundle branch block and left anterior fascicular block. These findings are compatible with an idioventricular rhythm focus in the left posterior fascicle.
