The ECG shows sinus tachycardia at 100 beats per minute with a PR interval of about 0.16 sec. There are Q waves in V1-4 with 6- to -8 mm ST segment elevations consistent with an extensive acute anterior ST-elevation myocardial infarction (STEMI). This "tombstoning" of the ST segment may occur during a large acute anterior MI.

The changes in this ECG are usually associated with thrombotic occlusion of the proximal left anterior descending coronary artery since the septal leads (V1 and V2) are involved. Also, there are lesser ST elevations in the inferior leads, II-III-AVF. Some left anterior descending coronary arteries “wrap around” the cardiac apex and supply part of the inferior wall as well, which was the case in this situation.
