The rhythm is sinus with occasional premature atrial beats and left atrial abnormality. The frontal plane axis is –30° and the R wave in AVL is greater than 20 mV (note standardization deflections are included in the tracing). The T waves are inverted or flattened in I-AVL-V5-6. These findings are consistent with left ventricular hypertrophy and repolarization changes.
