This ECG shows sinus rhythm at about 90 beats per minute. There are QS complexes in V1-2 and dramatic global down-sloping ST depressions consistent with ischemia. 

ST segment elevation in lead aVR greater than the ST elevation in lead V1 coexisting with ST depression in the precordial leads has been proposed as an ECG indicator of left main coronary occlusion; however, this has recently been questioned.
