This ECG shows sinus rhythm at about 75 beats per minute with some variability in the R-R intervals. The P wave is M-shaped in lead II with deep terminal inversion in V1.

The second QRS is a wide-complex premature beat. Note the morphology of the premature complex in lead V1. The R is greater than the R' with a right bundle branch block pattern, which is one of the Brugada criteria for a ventricular origin of a QRS complex.
