The ECG shows sinus rhythm at 100 to 110 beats per minute with an isolated (conducted) premature atrial contraction and a ventricular couplet. The QRS duration is about 0.15 sec and the frontal plane complexes show wide divergence of initial and terminal vectors. The chest leads show a tall RV1 and deep SV6. The findings are consistent with sinus tachycardia, right bundle and left anterior fascicular block, and ventricular ectopy.
