  1. Sinus tachycardia
  2. Right bundle branch block
  3. Left anterior fascicular block
  4. Anterior myocardial infarction

Recall that myocardial ischemia can indeed be interpreted in the setting of a right bundle branch block, but is more difficult in a left bundle branch block (although sometimes possible). Also, recall that the combination of a right bundle branch block with a left anterior or posterior fascicular block is called a "bifascicular block". When a first degree AV block is present as well, the term "trifascicular block" is also used.
