  1. Sinus tachycardia

The ST elevation on this ECG is much more subtle than frequently seen during an anterior MI which can cause "tombstoning" (see review below), however there is 2 mm of ST elevation in leads V2 and V3. This represented an acute occlusion of the left anterior descending coronary artery in the mid portion, after a major septal branch (thus V1 is not involved) and after a large diagonal branch (thus there is no lateral extension making V5 and V6 appear normal).
