The ST segment elevation on this ECG is much more subtle than that frequently seen during an anterior MI, which can cause tombstoning,”discussed in the Anterior Wall ST Segment Elevation MI ECG Review. However, there is 2 mm of ST segment elevation in leads V2 and V3. This represented an acute occlusion of the left anterior descending coronary artery in the mid-portion, after a major septal branch (thus V1 is not involved) and after a large diagonal branch (thus there is no lateral extension, making V5 and V6 appear normal).
