The ECG shows sinus rhythm with a rate of about 90 beats per minute. There are very prominent U waves, best seen in lead II.

The ECG manifestation of hypokalemia is a “U wave,” which is a positive deflection following the T wave (and before the next P wave) as seen on this ECG. This patient had a serum potassium level of 2.2 mEq/L.
