The ECG shows atrial fibrillatory activity, best seen in the V1 rhythm strip. There are no P waves. The ventricular complexes are irregularly irregular with a rate in the 60 beats per minute range, a controlled response to atrial fibrillation. There are “reverse check mark” ST-T wave changes consistent with digitalis effect in leads V5-6.
