The ECG shows sinus tachycardia with a rate of about 115 beats per minute (bpm). There is a S1-Q3-T3 pattern of acute right heart strain (McGinn-White sign) that is associated with pulmonary embolism. The T waves are also inverted in V1-4.

A recent review and meta-analysis that included more than 3,000 patients with acute pulmonary embolism demonstrated that six ECG findings of right ventricular strain were associated with increased mortality: heart rate > 100 bpm, the S1Q3T3 pattern, complete right bundle branch block, inverted T waves in V1–V4, ST elevation in aVR and atrial fibrillation.
