"An evaluation of his history, physical exam and workup. I attributed the elevated blood eosinophil counts, two history of severe asthma, atopic dermatitis and allergic rhinitis.

Despite his history of frequent sinus infections and pneumonia. I had low suspicion for common variable immunodeficiency due to normal levels of immunoglobulins and appropriate antibody response to vaccination.

There was low suspicion for allergic bronchopulmonary aspergillosis or ABPA due to normal IgE level and imaging not consistent with ABPA. He was referred to pulmonologists for their evaluation of the granuloma, and they recommend observation for now and repeat imaging in the future."
