Novel endoscopic platform ‘markedly’ facilitates ESD, reduces procedure time

WASHINGTON ─ A researcher from Baltimore presented a new endoscopic platform that improved removal of lesions and reduced procedure time here at Digestive Disease Week 2015.

Currently, “we do not have a stable view field … do not have traction or counter traction; … that makes it difficult,” said Sergey Kantsevoy, MD, PhD, of Mercy Medical Center in Baltimore.

He said endoscopic submucosal dissection (ESD) allows en block removal of premalignant and early malignant GI tract lesions; however, “ESD is technically difficult, time-consuming procedure, which so far has not been widely accepted in Western countries.”

Kantsevoy said the aim of the multicenter study was to evaluate the feasibility, safety and efficacy of a new endoscopic platform.

The LumenR Retractor (Macroplata, Inc.) system is “a portable chamber which is mounted on a flexible overtube  [and] you can open and close the chamber itself,” Kantsevoy said. “In addition, the LumenR has independent instrument guides which can deliver any type of instrument – grasping forceps, grasping snares, [and] so forth.”

“The procedure closely resembles laparoscopic surgery.”

Also, “the size of the lesion is not limited to the size of the chamber.”

Kantsevoy said LumenR has been used in 34 patients. The mean age of the patients was 61 years and most were men.

En block resection was successful in 100% of the patients with mean procedure time of about one hour and 30 minutes, he said. The procedure to remove the largest lesion (130 mm) took 2 hours and 30 minutes.

In a typical case, the LumenR needed only one physician and one assistant.
