Cold snare technique superior in diminutive colonic polyp removal

PHILADELPHIA — Cold snare polypectomy or jumbo biopsy techniques were superior to standard cold biopsy in the removal of diminutive colonic polyps, according to data presented at the ACG Annual Scientific Meeting.

“The aim of our study was to compare the complete histological eradication rate of diminutive colonic polyps using cold forceps biopsy vs. any other…method,” Dany Raad, MD, of Case Western Reserve University, University Hospitals Case Medical Center, and the VA Medical Center in Cleveland, said in a presentation.

In March 2014, Raad and colleagues performed a systematic review and meta-analysis of five randomized controlled trials including 610 patients, in order to compare complete eradication rate of diminutive polyps and procedure time with cold biopsy vs. jumbo biopsy and cold snare polypectomy.

Average polyp size was 4.5 mm (range, 2-10 mm). Analysis of histological criteria revealed that reduced incomplete polyp eradication was associated with cold snare or jumbo forceps biopsy compared with cold biopsy (RR=0.48; 95% CI, 0.3-0.77). Procedure time was also reduced by 4.1 minutes on average with cold snare or jumbo forceps biopsy compared with cold biopsy (95% CI, –8 to –0.2).

“In conclusion, we can say that there is moderate quality evidence that using [cold snare or jumbo biopsy] reduces your risk of incomplete diminutive colonic polyp removal by 52%, as compared to the cold forceps biopsy without increasing your procedure time,” Raad said.

“Of course further studies are needed to determine the most accurate method of removal of those polyps.”
