Ace the Case: A 24-Year-Old Female with Chronic Low-Back Stiffness
Paula is a 24-year-old Caucasian female who presents with 3 years of chronic low-back stiffness. Her symptoms began following her fourth pregnancy 3 years ago and were initially treated with acetaminophen without any relief. She reports a daily aching sensation of stiffness in her lower back and buttocks that improves with activity and awakens her from sleep at least 3 times per week. Over the last month, she noted the development of diffuse abdominal cramping, bloating, and 3 to 4 blood-streaked bowel movements per day. She initially attributed the symptoms to taking ibuprofen 800 mg 3 times daily to control her pain, but her gastrointestinal symptoms persisted for the past 3 weeks despite discontinuation of ibuprofen.
