I think this is a great question. I don't think one size fits all, and I do individualize therapy for our patients, and I think so many factors become important to that question. How frail somebody is, what other comorbid conditions do they have? You know, would they have a poor outcome with a certain treatment? Do I need to dose reduce it? Do I need to adjust the combination that I would normally do, right?
What different organ systems are getting involved? Is it, for example, in vasculitis, is it just the kidneys? Are the lungs involved? Is it sinuses getting involved? What degree of involvement is there? You know, are there other infections? So different factors that affect, you know, the decision to treat patients.
However, the most important factor is patient choice, right? This is not a decision that I take. This is a combined decision that I have with my patients. We have a detailed discussion about risks and benefits with these treatment options, and then we figure out what's best for them.
