Demonstrated in this video are the anterolateral and posterior paramedian approaches for fixation of rib fractures, which are a component of flail chest injuries.

Niloofar Dehghan, MD, FRCSC, chief of orthopedic trauma at The CORE Institute in Phoenix, and Aaron Nauth, MD, FRCSC, an orthopedic trauma surgeon and assistant professor at the University of Toronto, present both approaches and the fixation, including direct reduction followed by plate fixation of the rib fractures.

Also shown are plate contouring and a small thoracotomy used to access the plural space for irrigation and evacuation of any retained hemothorax.
