Frequency of THA increased in 2 decades among patients younger than 21 years of age

Published results showed an increase in the number of total hip arthroplasties performed in patients younger than 21 years of age in the United States between 2000 and 2016.
Bella Y. Mehta, MBBS, MS, of Hospital for Special Surgery, and colleagues retrospectively reviewed the Kids’ Inpatient Database for patients younger than 21 years of age who underwent elective and non-elective primary THA between 2000 and 2016. Researchers considered the frequency of THA performed in these patients as the primary outcome. Researchers utilized ICD-9 and ICD-10 codes to determine primary diagnoses.
Results showed an increase in the weighted total number of THAs performed in patients younger than 21 years of age from 347 procedures in 2000 to 551 procedures in 2016, with the most common diagnoses being osteonecrosis, osteoarthritis and inflammatory arthritis. Researchers found an increase in the frequency of THA for osteonecrosis from 24% in 2000 to 38% in 2016. However, the frequency of THA for inflammatory arthritis decreased from 27% in 2000 to 4% in 2016, according to results.
Researchers noted an increase in the percentage of THAs performed in urban teaching hospitals, with 80.3% of THAs performed in urban teaching hospitals, 17.3% in urban non-teaching hospitals and 2.4% in rural hospitals during the study period. Researchers found a decrease in length of hospital stay in patients younger than 21 years of age undergoing THA from a mean of 7.3 days in 2000 to 3.2 days in 2016. Results showed 53.1% of patients were discharged home and 34.8% of patients were discharged home with home health care services.
“In this nationwide analysis of total hip arthroplasty utilization in patients under 21 years of age, we found that the number of THAs performed in this population increased from 2000 to 2016, despite a decrease in the use of THA for inflammatory arthritis,” Mehta told Healio Orthopedics. “Knowledge of the epidemiology of THA utilization in this population may help to guide health policy and future research into the indications, outcomes and failure modes of THA in young patients.”
