No differences seen in outcomes after conventional vs custom instrumentation TKA

Custom instrumentation compared with conventional instrumentation for total knee arthroplasties did not decrease operative time, transfusion rate, postoperative hemoglobin drop, hemovac output, or hospital length of stay, according to recently published results. Custom instrumentation also did not change the discharge disposition or decrease the discharge rate to a rehabilitation facility.
Researchers compared 231 conventional TKAs with 231 custom instrumentation TKAs. The custom instrumentation TKAs were all performed using The MyKnee (Medacta) CT-based femur and tibia system. Transfusion rate, hemoglobin drop, hemovac output, operative time, length of stay, discharge disposition, deep venous thrombosis and pulmonary embolism and in-hospital vital sign data were among the perioperative factors assessed.
Results showed conventional TKAs and custom instrumentation TKAs were no different regarding operative time, transfusion rate, discharge hemoglobin, length of stay, discharge disposition or in-hospital venous thromboembolism rates. The length of stay in patients who underwent conventional TKA was 2.6 days vs. 2.5 days for patients who underwent custom instrumentation TKA. The discharge disposition was 82% home for patients who underwent conventional TKA and 83% home for patients who underwent custom instrumentation TKA. – by Monica Jaramillo
