VIDEO: Cervical total disc replacement

In this video from the Open Operating Theatre, Nils Hansen-Algenstaedt, MD, of OrthoCentrum Hamburg in Hamburg, Germany, discussed how to perform cervical total disc replacement in a 36-year-old woman.

The patient is placed is supine position with a Mayfield headset and neck support. A lateral X-ray is performed to ensure the next level. Skin incision is done transversely. After medial retraction of the esophagus, hemostasis is prepared. After lateral X-ray control, two pins are placed parallel to vertebral endplates. The C4-C5 disc is removed and the upper and lower plates are cleaned. Distractors are placed to open the disc plate. A large deficit is seen in the ligament and is excised carefully to avoid dural injury or leak. The dural is exposed followed by spinal cord decompression. The disc prosthesis is inserted, and optimal position is checked. The fascia and skin are closed.
