In this video from the Open Operating Theatre, Hossein Mehdian, MD, of Queen’s Medical Centre, Nottingham U.K., discusses how to perform a posterior spinal fusion and how to correct Scheuermann kyphosis.

Surgery begins with the patient placed in a prone position. A posterior midline incision is made from T1-L4 and a subperiosteal dissection is performed. Pedicle screws are placed bilaterally from T2-L3. The spinous processes, supraspinous and intraspinous ligaments are removed before the Ponte osteotomy is performed at T4-T5, T6-T7, T7-T8 and T8-T9. Laminotomy, flavectomy and a facetectomy are used to complete the osteotomy. Contoured rods are then inserted and fixed to the pedicle screws. Bone graft and bone graft substitute is then placed and used to achieve spinal fusion. Surgeons then close the wound.

This video is intended as a teaching tool to help surgeons prepare for surgery or they can use it as a refresher on the surgical technique and indications for this surgery.
