Variable outcomes seen after arthroscopic posterior labral repair in patients older than 35 years

Investigators found patients who were older than 35 years had variable outcomes after arthroscopic posterior labral repair and linked chondral damage at the time of surgery with worse results in this cohort.

Researchers performed a retrospective review of information on 43 patients older than 35 years who underwent arthroscopic posterior labral repair. The minimum follow-up was 24 months, with an average follow-up of 36.9 months. At 2 weeks, 6 weeks, 3 months and 6 months, patients underwent follow-up assessments that included QuickDASH, the Simple Shoulder Test, the Western Ontario Stability Index (WOSI) and the Single Assessment Numeric Evaluation (SANE). Patients were contacted either via telephone or mail and were given a questionnaire to complete about their operative shoulder if their follow-up exceeded 24 months.

The impact of preoperative symptomatic instability, injury mechanism, workers’ compensation claims and intraoperatively noted chondral damage were determined with subgroup analyses. Investigators recorded complications and whether revision surgery was needed.

Results showed no significant differences in outcomes regarding preoperative symptomatic instability, active workers’ compensation claims or traumatic injury. Investigators noted intraoperatively defined chondral damage correlated with worse functional outcomes in patients by way of QuickDASH, the Simple Shoulder Test, WOSI and SANE scores. A minor complication was seen in one patient, while three patients required additional surgery that included two total shoulder arthroplasties and one revision labral repair. by Monica Jaramillo
