No advantages seen with closed suction drainage for direct anterior hip arthroplasty

Results from this study indicated patients who underwent direct anterior total hip arthroplasty with closed suction drainage had no clinical benefits or disadvantages compared with patients who had procedures performed without closed suction drainage.
“This study demonstrated no significant benefits, but also no disadvantages, with the use of closed suction drains in a select group of patients undergoing direct anterior total hip arthroplasty for whom tranexamic acid was used perioperatively and aspirin was used for [venous thromboembolism] VTE prophylaxis,” Juan Suarez, MD, an orthopedic surgeon at Cleveland Clinic, Florida and principal investigator of the study told Healio.com/Orthopedics. “The use of drains did not show an advantage in preventing wound complications or less surgical site swelling. In contrast to historical data, the use of drains did not lead to a higher blood loss or higher transfusion rate in our study population.”
Suarez and colleagues prospectively randomized patients who underwent direct anterior total hip arthroplasty to receive either closed suction drains or no drains. Hematoma formation, wound complications and transfusion rates were the primary outcomes of the study. Investigators also estimated blood loss, looked at decrease in hemoglobin levels and hematocrit levels, total hemoglobin loss, calculated blood loss, hidden blood loss and total length of hospital stay.
Investigators found patients with closed suction drains and no drains were not significantly different regarding the transfusion rates, postoperative decrease in hemoglobin levels, average calculated blood loss, complications and length of hospital stay.
