Re-application of surgical site preparation solution linked with reduced SSIs in TJA

Recently published results showed a significant reduction in the rate of superficial surgical site infections when surgical site preparation solutions were re-applied after draping and before the application of iodophor-impregnated incisive draping among patients who underwent total joint arthroplasty.
Researchers randomly assigned 600 patients undergoing total joint arthroplasty (TJA) between 2010 and 2011 to either an intervention group, which received a surgical site preparation solution that included alcohol and povidone-iodine before draping and an additional surgical site preparation solution by iodine povacrylex and isopropyl alcohol before application of the final adhesive drape, or to a control group, which received a single surgical site preparation solution with alcohol and povidone-iodine before draping. The final analysis included information for 577 patients.
Results showed a total of 29 surgical site infections among all patients, with a statistically significantly lower incidence of superficial surgical site infections and a lower, but not statistically significant, incidence of skin blistering in the intervention group vs. the control group. Researchers noted the intervention group had two cases of organ-space surgical site infection, while the control group had two cases of deep surgical site infection.
When stratified by joint type, patients in the intervention group undergoing either total hip arthroplasty (THA) or total knee arthroplasty (TKA) had a lower rate of superficial surgical site infection and less blistering vs. the control group, according to results. Compared with controls, researchers also noted the intervention group had lower rates of deep/organ space surgical site infection among patients who underwent THA and found one deep/organ space surgical site infection among patients who underwent TKA.
