Anterior knee pain has some characteristics that need to be carefully considered. Despite the fact that anterior knee pain can be caused by numerous conditions, not all patients have an underlying identifiable patellofemoral disorder. Also, meticulous clinical examination may hold the key to a specific diagnosis. Rehabilitation can significantly improve anterior knee pain and it is proposed as the initial step in most patients. When symptomatic treatment fails, careful patient selection for operative management is critical. The most important characteristics to guide the choice of surgical procedure are the manifestation of load-related pain and the presence of instability. When performed according to the criteria described above, operative management can offer an exceptional long-term outcome.
