Young men at highest risk for closed reduction after elbow dislocation
Men 20 years of age or younger are at the highest risk of closed reduction following an elbow dislocation, according to recently published data.
Investigators culled information from administrative databases on 4,878 patients ages 16 years and older (median age, 41 years) who underwent closed elbow reduction between 1994 and 2010. The closed reduction incidence density rate (IDR) per 100,000 person-years was analyzed and a multivariate logistic regression model for failure was constructed. Failure was determined as subsequent open reduction with or without ligament repair or reconstruction within 90 days postoperatively.
The IDR of patients who underwent closed reduction was 2.65 per 100,000 person-years. IDR was highest in men 20 years or younger (7.45), double that of women in that age group, according to the investigators.
Overall, 75 patients (1.5%) were classified as having undergone failed closed reduction and underwent open reduction within a median of 15 days postoperatively. Failed closed reduction was most likely in patients who were older, had been admitted to the hospital, had more than one previous closed reduction attempted and had an orthopedic consultation within 4 weeks following a closed reduction, according to the investigators. – by Christian Ingram
