Nearly 95% of patients return to sports after Bankart procedure
A new study with more than 20 years follow-up shows that the Bankart operation may be highly effective for patients with shoulder instability who compete in contact sports.
In a retrospective study, Alain Durandeau, MD, and colleagues followed 49 patients who were treated for shoulder instability with a Bankart procedure for a mean of 26 years. Eighty percent of the patients participated in competitive sports, and of these, 72% played contact sports.
The investigators discovered that 94% of the patients returned to sports at an average of 4.6 months postoperatively, and 80% returned at the same playing level. They also found an average deficiency of 9° external rotation compared to patients' nonoperated, contralateral side, and a recurrence rate of 16%.
All recurrences were provoked by a violent sports accident, said Durandeau, who presented the study at the 9th European Federation of National Associations of Orthopaedics and Traumatology Congress in Nice.
Patients underwent surgery at a mean age of 25 years, and an average of 4 years elapsed from the time of the first dislocation to surgery. Surgeons immobilized the patients postoperatively and prescribed self-controlled rehabilitation.
Long-term radiographic evaluation of the 50 shoulders showed that 26% had normal radiographs. Using the Samilson and Prieto classification, the investigators found that 18 patients had Stage I osteoarthritis and five patients had Stage II. They classified five patients as Stage III and only one patient as having Stage IV osteoarthritis.
"In this cohort with 80% competition-sports athletes and 94% return-to-sports activities after surgery, the Bankart repair operation demonstrated its efficacy for contact sports," Durandeau said. "The rate of osteoarthritis after this operation is comparable with that observed with other types of bone blocks, but the follow-up was twice as long. The deficit in external rotation was not greater than that with non-anatomic operations."
