Mid-vastus approach effective, safe for TKA

The mid-vastus approach could be a viable alternative to the medial parapatellar approach when performing primary total knee arthroplasties, according to researchers in the United Kingdom.

The mid-vastus approach offered decreased lateral release rates and earlier rehabilitation, the authors noted, with no increase in complication rates.

In a meta-analysis of randomized and quasi-randomized controlled trials that adhered to Cochrane Handbook for Systematic Reviews of Interventions guidelines, the researchers looked at 18 studies involving 1,040 patients. Average patient age was 69 years for the mid-vastus group and 68.9 years for the medial parapatellar group.

The mid-vastus group experienced significant improvements in flexion and visual analogue scale score throughout the first postoperative week, as well as a reduction in the number of required lateral releases. The authors also reported no increase in complication rates.
