Because nearly 87% of patients who present for cataract surgery have some amount of astigmatism, knowledge of the astigmatic tolerance of IOLs is important in surgical planning, the study authors said. They evaluated the effect of induced astigmatic defocus in 40 eyes of 40 patients equally divided into two groups implanted with the Tecnis Eyhance DIB00 IOL or the Tecnis ZCB00 monofocal IOL (both Johnson & Johnson Vision). Astigmatic defocus was induced over the manifest distance refraction with plus cylinder from +0.5 to +2 D in 0.5 D steps for each astigmatic orientation, ie, with-the-rule (WTR), against the-rule (ATR) and oblique.
While both IOLs showed similar tolerance in WTR orientation, the Eyhance IOL showed better tolerance to induced astigmatism over the entire diopter range in the ATR orientation and oblique orientation, which are the two orientations that cause more degradation in visual acuity.
“This provides insights into the potential role of monofocal IOLs with extended depth of focus features in achieving greater tolerance to residual refractive error,” the authors wrote.
No statistically significant difference was found in distance visual acuity with the two lenses, but intermediate vision and near vision were better with the Eyhance IOL compared with the standard monofocal lens. After statistically controlling for pupil size, these findings remained consistent.
