Trial assessed whether asymptomatic embolic signals detected with transcranial doppler were predictive of stroke in patients with asymptomatic carotid stenosis.

Design: prospective, observational
Patients: 482
Centers: 26
Countries: international

RESULTS: There were 32 incidences of the primary endpoints during follow-up (26 ipsilateral TIA and six ipsilateral strokes). The hazard ratio for the risk for ipsilateral TIA and stroke in patients who displayed embolic signals vs. those who did not was 2.54 (95% CI, 1.20-5.36), and was 2.39 (95% CI, 1.12-5.11) after controlling for the presence of antiplatelet therapy at baseline. Absolute annual risk of ipsilateral TIA between baseline and two years was 7.13% in patients with embolic signals vs. 3.04% in patients with no signals, and was 3.62% for ipsilateral stroke in patients with embolic signals vs. 0.70% in those without signals.
