Advance Care Planning

Patients with breast cancer given personalized care sequence plans took more efficient and timely action on key activities such as primary care, testing and appointments, before receiving treatment or surgery compared with patients without care sequence plans, according to research presented during ASCO2020 Virtual Scientific Program.

Through a partnership with the Pathways Project, in collaboration with the Coalition for Supportive Care of Kidney Patients, a Dallas-based nephrology practice has identified ways to tailor advance care planning to the individual patient. The methods, which assessed using “small tests of change,” were presented at the virtual National Kidney Foundation Spring Clinical Meetings.

A published study suggested that a single-item question may help providers better understand the treatment preferences of patients with advanced chronic kidney disease and enable more effective discussions about patient goals.
