LAS VEGAS — Joe Parks, PhD, medical director at the National Council for Behavioral Health, discusses how community mental health centers in Missouri used a population health management model to integrate health care services and improve health care experiences and outcomes for individuals.
Parks and colleagues decided to remodel their health care approach form the usual complaint-driven model due to the complexities, difficulties and barriers that model poses for individuals, particularly those with multiple, chronic conditions.
“People with serious mental illnesses usually have two or three mental illnesses, three or four medical conditions, often seeing a dozen to 15 different providers in a year, and often taking a similar number of medications. Yet we expect a person whose illness we know causes difficulty with memory, concentration, perseverance on task and organizing, to keep track of all these things,” Parks told Healio.com/Psychiatry.
