These photos of an asymptomatic 71-year-old female were chosen as the 2012 annual grand prize winner of the Primary Care Optometry News “Ocular Photography Challenge.” Lee Peplinski, OD, FAAO, of Louisville, Ky., said the patient had a rather extensive ocular history including high myopia, bilateral cataract surgeries and retinal detachment with a vitrectomy/scleral buckling procedure. A subsequent detachment with proliferative vitreoretinopathy resulted in another vitrectomy with placement of silicone oil.

The patient’s vision in this eye never improved to better than hand motion or finger counting at a few feet after the original detachment and it remains at this level, Dr. Peplinski said. The images show approximately 2 mm of layered, emulsified oil droplets collecting in the superior angle of the anterior chamber. Her IOP has remained 12 mm Hg to 14 mm Hg on several visits (despite the obstructed angle), and her retina remains fully attached. As such, her retinal surgeon advised only continued monitoring. It has now been more than 18 months since these photos were taken, and her eye remains stable. These photos were taken with a Hitachi HV-D30 camera mounted in the EyeRes 2000 system (TelScreen).

As the annual grand prize winner for 2012, Dr. Peplinski will receive a pair of Genesis binoculars, provided by Kowa.
