ST. LOUIS — The National Optometry Hall of Fame is now accepting nominations for 2012, according to a hall of fame press release.

An ideal candidate for nomination would be widely recognizable through his or her national stature and also have had significant and enduring impact on the optometry profession. When presenting a nominee for selection, the nominee’s full range of contributions should be represented, including examples of his or her professional leadership, academic leadership and research contributions, according to the release.

Organizations included in the selection committee are the Association of Schools and Colleges of Optometry, the College of Optometrists in Vision Development, the National Optometric Association, the American Optometric Association and the American Academy of Optometry. Inductees will be honored at Optometry’s Meeting in June 2012 in Chicago.

The 2012 nomination forms can be downloaded from www.aoa.org/x18530.xml or by emailing a request to Foundation@aoa.org with “2012 Nomination Form” as the subject line. Nomination forms and supporting documentation for candidates should be sent to the National Optometry Hall of Fame at 243 North Lindbergh Blvd., St. Louis, MO 63141. Email submissions should be sent to Foundation@aoa.org.

Deadline for nominations is Dec. 1.
