This photo of a 63-year-old white female with a history of retinal detachment and scleral buckle repair in December 2007 in the left eye was chosen as the winner of this month’s “Ocular Photography Challenge.” Sherry L. Winn, OD, of Clarks Summit, Pa., said the patient recently called complaining of the development of a small subepidermal hemorrhage temporally on the lower left lid upon awakening. She was advised to come in due to her history.

Slit-lamp exam revealed an exposed scleral buckle superior temporally. Ironically, the patient was seen just 3 days prior by the retinal specialist for a follow-up and by me that same afternoon for a dry eye-related appointment. Neither the retinal specialist nor I noted any abnormalities on that day.

I immediately contacted the retinal specialist regarding the situation and was advised to monitor the patient closely for development of conjunctival irritation. If the exposed buckle causes chronic irritation or inflammation it will have to be removed.

The patient was seen back 1 week later and the buckle exposure increased slightly. The patient will be monitored closely over the next year to rule out both external and internal complications.

The photos were taken with the Telscreen platinum series slit-lamp camera.

As this month’s winner, Dr. Winn will receive an HA-2 Handheld Tonometer, provided by Kowa.
