Well, you need to screen patients once they enter stage 3 CKD. The guidelines recommend doing it at least annually and as much as quarterly or even monthly if you're treating them as you get into stage 4 and 5 CKD. If you don't look, you're not gonna find it.

If you look at all the patients who have CKD stage 3, 4, or 5, most of whom are in stage 3, about a quarter of them are anemic, and even more, 50-70%, depending on whether they're men or women, are actually iron deficient. So, if you're also looking for iron deficiency, you'll find it more frequently than you find anemia from iron deficiency.
