This year marked a terrific Transcatheter Cardiovascular Therapeutics (TCT) meeting in Washington, D.C. Attendees from all over the world gathered to hear about the latest developments in interventional cardiology and other related areas.
Perhaps the most-awaited trial was EXCEL, which randomly assigned patients with left main CAD to either CABG or stenting, and was presented at TCT 2016 as well as published in The New England Journal of Medicine. That trial showed that both approaches to left main disease had merit. Another trial, NOBLE, was also presented on this topic and provided complementary information. In aggregate, the data seem to support that for some patients stenting would be better, and in other patients CABG may be better. However, nuanced discussions with the patient and involving the heart team will often be necessary.
This issue of Cardiology Today’s Intervention provides a variety of perspectives on the optimal strategy for left main disease, as well as other hot topics from TCT 2016 such as additional real-world data regarding left atrial appendage closure and patent foramen ovale closure. There have been a number of developments both in terms of new data and regulatory changes with respect to such devices.
