Women face higher mortality risk after PCI for STEMI

LAS VEGAS — At 30 days, women with STEMI who underwent primary PCI had a mortality rate that was more than double that of men, according to data presented at the Society for Cardiovascular Angiography and Interventions Scientific Sessions.
Francesco Tomassini, MD, of Infermi Hospital, Rivoli, Italy, and colleagues analyzed 1,251 patients (313 women) who underwent primary PCI within 12 hours of STEMI at their institution between September 2001 and June 2010. They compared 30-day mortality rates and a number of clinical and procedural characteristics between men and women.
The 30-day mortality rate was 11.8% for women vs. 5.5% for men (P<.0001). After propensity adjustment, women remained at an elevated risk for 30-day mortality compared with men (OR=0.56; 95% CI, 0.34-0.93).

The risk for bleeding at 30 days was also significantly higher among women: 4.5% vs. 1.4% for men (P=.002).
“Our data suggest the hypothesis that biological gender-related differences could, in part, explain these findings,” Tomassini and colleagues wrote in the abstract.
According to other results, women had a higher symptom-to-balloon time (291 minutes vs. 238.5 minutes; P<.0001) and lower left ventricular ejection fraction (46.8% vs. 48.5%; P=.007) compared with men. Further, the researchers observed, women were less likely compared with men to be treated with glycoprotein IIb/IIIa inhibitors (76% vs. 85.8%; P<.0001), thrombus aspiration devices (42.4% vs. 57.3%; P<.0001) or stents (81.1% vs. 88.4%; P=.002).
Compared with men, women were less likely to smoke (24.9% vs. 75.7%; P<.0001), less likely to have left main disease (3.8% vs. 5.1%; P<.0001), more likely to have diabetes (32.4% vs. 24%; P<.0001) and more likely to have hypertension (77.3% vs. 65.8%; P<.0001). – by Erik Swain
