INDIANAPOLIS — An aceclidine-based miotic may improve both near and distance vision in presbyopic patients as a result of its pupil selectivity, according to research presented at Academy 2024.

For patients with presbyopia, it depends on factors such as age of the patient or if there is any other pathology.

Small-diameter aperture optics, or as many call it, the pinhole, can increase depth of focus, enhance quality of vision at distance in select patients and mask small refractive errors, whether myopia, hyperopia, astigmatism or presbyopia.

A pinhole effect, achieved through surgery, pharmacologic agents, IOLs or inlays, can treat significant irregular corneal astigmatism and higher-order aberrations, whose effects are minimized by reducing pupillary aperture.

An estimated 128.5 million people are expected to be presbyopic in the United States by 2023.

Two main classes of pharmaceutical therapies that use different mechanisms of action to treat presbyopia may be effective, noninvasive solutions for patients who are not yet good candidates for surgery.
