ORLANDO, Fla. — Rheumatology nurses from the Medical College of Wisconsin implemented a program to help their patients transition from pediatric to adult care, according to data presented here at the Rheumatology Nurses Society Annual Conference.

Betsy Roth-Wojcicki, MS, CPNP, and colleagues administered the “Got Transition” model and a survey to 120 patients aged 16 years or older. The transition program consisted of an introductory letter, a self-assessment questionnaire, a personalized educational transition binder, counselling, a check list, an individualized summary pamphlet and an app. They found patients learned best from hand-on learning, followed by listening, reading, pictures and online. In addition, staff found the model easy to implement and user friendly. They concluded administrative support is essential for transition. — by Will Offit
