Gout factors have stronger association with age than gender

Factors associated with gout and previously linked to women with the disease appear more connected to age rather than gender, according to study results.

Researchers in Mexico evaluated 37 women (mean age, 54.47 years) and 37 men (mean age, 53.52 years) diagnosed with gout (mean age at onset, 46.77 years and 45.62 years, respectively). Renal function — measured by creatinine clearance, modification of diet in renal disease (MDRD) and Cockcroft-Gault method — clinical data and associated diseases were considered variables.

Men had more frequent podagra (91.9% vs. 59.5%, P=.001), erythema during acute attack (88.2% vs. 22.2%, P=.000) and rapid onset of pain and swelling (<12 hours, 70.6% vs. 33.3%; P=.028) than women. No significant differences were found between the sexes regarding hypertension, arthritis of the upper limbs, annual acute gout attacks, familial history of gout, tophaceous gout or associated metabolic diseases. Creatinine clearance levels were lower in women compared with men (49.8 [29.7] mL/min vs. 67.1 [35.5] mL/min, P=.039). No significant differences were observed, however, when adjusted for sex (Cockcroft-Gault, 66.4 [37.6] mL/min per 1.73m2 vs. 78.8 [43.8] mL/min per 1.73m2 or MDRD, 73.8 [64.6] mL/min per 1.73m2 vs. 73.1 [35.0] mL/min per 1.73m2).

Among women, 35.1% were premenopausal at disease onset, with two patients having family histories of gout. At onset, 15.4% of premenopausal women had lithiasis while no postmenopausal women had the condition (P=.048).

“When females and males with diagnosis of gout are paired by age and duration of disease, there are no significant differences in the clinical data related to gout and associated disease or when the renal function evaluation is calculated with methods that include sex in the equation,” the researchers concluded.
