16-month-old female is admitted to PICU with concerns for sepsis
A 16-month-old female is admitted to the PICU with fever and concerns for sepsis. A few weeks earlier, she had the onset of a generalized maculopapular rash. The rash seemed to wax and wane, and then 4 days prior to admission, she was seen and treated with oral amoxicillin for fever and acute otitis media. The rash had since transitioned into several dark skin lesions with some surrounding erythema. Several lesions had become significantly larger, especially a presacral lesion (Figure 1), as well as lesions on both upper and lower lips (Figure 2) and her right ankle. A swab of the surface of the presacral lesion was sent for routine bacterial culture, growing three organisms: methicillin-sensitive Staphylococcus aureus (MSSA), Enterobacter and Klebsiella.

A 3-year-old male develops foot pain, gingival edema
A 3-year-old male living in Puerto Rico presented with progressive bilateral foot and lower extremity pain. He had been healthy until 4 months earlier, when he was diagnosed with influenza B. In addition to the common symptoms of influenza at that time, he also had the onset of pain in both feet. This pain continued, with a pattern of getting worse in the mornings, and reached a point where the patient refused to wear shoes. An orthopedic consultant failed to find a problem, and the patient was treated with pain medication, which had minimal effect. As his pain continued, he began having painful gingival swelling and erythema. His dentist referred him to a local hospital, where he was diagnosed with gingivitis and myositis, supported by an elevated CPK of 1760 and an ESR of 40, with a negative bone scan. The patient’s gingival edema worsened, with friable bleeding gums and ongoing leg pains, and he was taken to the regional medical center. Additional history revealed that he never had fever, night sweats or joint inflammation or swelling, but he did have a mild, transient rash, now resolved.

NEW YORK — Infectious Diseases in Children Editorial Board Member Lawrence F. Eichenfield, MD, chief of pediatric and adolescent dermatology at Rady Children’s Hospital and vice chair of the department of dermatology at the University of California, San Diego, presented a live version of “Spot the Rash” at the Infectious Diseases in Children Symposium.
Infant presents with severe diaper dermatitis
A 4-week-old male is being seen for a 3-week history of severe diaper dermatitis. The rash has been treated with clotrimazole and a barrier cream, with some improvement about a week ago. However, three discrete, erosive lesions subsequently developed, along with a low-grade fever of 100.6°F that was documented in the ED. Due to the fever, the baby was admitted to the hospital for a sepsis workup that included a blood culture; urinalysis and urine culture; a wound culture of the larger lesion; and a herpes simplex PCR. The baby was empirically started on therapy with ampicillin plus gentamicin. A lumbar puncture was attempted but was unsuccessful, and it was later deemed unnecessary and therefore was not reattempted.
Melanoma rates decline among US adolescents, young adults
Incidence of invasive melanoma decreased among adolescents and young adults in the United States between 2006 and 2015, according to results of a population-based study published in JAMA Dermatology.

BOSTON — Maralixibat demonstrated durable control of serum bactericidal antibody, pruritus, and xanthoma in children with Alagille syndrome, according to data from the ICONIC study presented at The Liver Meeting 2019.
