Key takeaways:

  • Diosmin reduced lower extremity swelling and pain during motion for patients who underwent TKA.
  • Diosmin was not associated with increased short-term complications compared with a control group.

According to published results, use of diosmin after total knee arthroplasty was associated with reduced lower extremity swelling and reduced pain during motion with no increase in short-term complications.

Researchers performed a randomized, controlled multicenter trial that included 309 patients who underwent TKA and were randomly assigned to either a diosmin group (n = 156) or a control group (n = 153). According to the study, the diosmin group received 0.9 g of oral diosmin twice per day for 14 consecutive days after surgery. The control group received no intervention.

Outcome measures included lower extremity swelling, VAS pain scores, Hospital for Special Surgery (HSS) scores, knee range of motion (ROM), C-reactive protein and interleukin inflammation levels, and overall complications within 14 days.

Compared with the control group, the diosmin group had significantly less swelling of the calf, thigh and patella. The diosmin group also had significantly lower VAS pain scores during motion vs. the control group. Researchers found no other significant differences between the groups in terms of VAS pain scores at rest, HSS scores, ROM, inflammation biomarkers or complications.

“Postoperative lower extremity swelling is a major hindrance to the enhanced recovery of patients undergoing TKA,” the researchers wrote in the study. “As indicated by the difference in swelling between the control and diosmin groups on postoperative days 1, 2, 3 and 14, the ability of diosmin to reduce swelling may reflect its vascular protective and anti-inflammatory effects,” they concluded.
