Augmented reality navigation system may be accurate, effective for TKA

Published results showed use of an augmented reality navigation system may be accurate and effective in performing total knee arthroplasty.
Researchers performed a systematic review of the PubMed, Medline and Embase databases up to May 2021 using keywords “augmented reality,” “knee arthroplasty,” “computer-assisted surgery” and “navigation knee arthroplasty.” Researchers also performed TKA among five patients using the Knee+ augmented reality navigation system (Pixee Medical) with M400 Smart Glasses (Vuzix Corporation). Researchers recorded the time of tourniquet and evaluated the axial alignment of the limb and the orientation of the components on standardized preoperative and postoperative full-leg length weight-bearing radiographs, anteroposterior radiographs and lateral radiographs of the knee. Researchers had the surgeon complete the virtual reality sickness questionnaire immediately after surgery to assess perception of motion sickness.
Overall, researchers identified 31 abstracts in their systematic review. However, only two studies concerned knee arthroplasty and both were preclinical studies, according to researchers.
Results showed the Knee+ system had a cutting error of less than 1° of difference in coronal alignment of the femur and tibia, as well as less than 2° of difference in flexion/extension of the femur and posterior tibial slope. Researchers found absolute differences of approximately 0.6° for varus femur, 0.8° for varus tibia, 0.8° for posterior slope and 0.4 mm for femur flexion angle between values obtained during surgery and measurements on postoperative radiographs.
“From our pilot study, the [augmented reality] AR system in knee arthroplasty may become a useful alternative navigation system,” the authors wrote. “The translation from pilot study to high-level prospective studies is warranted to assess accuracy, limitations and cost-effective analysis compared to conventional techniques.”
