Jonathan C. Levy, MD, of Holy Cross Orthopedic Institute in Fort Lauderdale, Florida, performs a functional spacer surgical technique to treat shoulder prosthetic joint infection.

The steps shown include canal reaming and broaching. Bone cement mixed with 2 g tobramycin and 3 g vancomycin is used. When it is moldable, the cement is added to the humeral stem of the implant and formed so that it creates a metaphyseal fit and the thickness of the cement is tapered from proximal to distal. The spacer is introduced gently into the humeral canal when the cement is at the doughy stage.

In a study of 31 patients treated with this hemiarthroplasty functional spacer technique, Levy and colleagues reported 74% of patients did not undergo a second-stage surgery.
