Supra-ilioinguinal approach may yield good outcomes for bicolumnar acetabular fractures

Data published in the Journal of Orthopaedic Trauma showed good reduction and fixation of bicolumnar acetabular fractures with few complications may be acquired using the supra-ilioinguinal approach, which provides direct visualization and convenient access to the quadrilateral plate.
Researchers identified 22 patients with bicolumnar acetabular fractures treated with a single supra-ilioinguinal approach between January 2012 and June 2015 with a minimum of 1-year follow-up. The researchers hypothesized that the single supra-ilioinguinal approach may lead to shorter surgical time, less blood loss and fewer complications.
Researchers found five anterior column with posterior hemi-transverse patterns, 11 associated both column patterns, three transverse patterns and three T-type patterns. Results showed patients had an average length of incision of approximately 10.7 cm, an average operative time of 182 minutes and an average intraoperative blood loss of approximately 793 mL. Using the Matta scoring system, researchers found 77%, 18% and 5% of reductions were graded as excellent, good and poor, respectively, with a statistically significant correlation between the reduction quality and the time from injury to operation. According to results, Merle d´Aubigné scores were excellent in 64% of cases, good in 27% of cases and poor in 9% of cases at the last follow-up. Researchers noted one patient experienced postoperative deep vein thrombosis and three patients experienced lateral femoral cutaneous nerve injury. – by Casey Tingle
