The underlying impingement in combined-type femoroacetabular impingement can be effectively addressed with isolated, arthroscopic acetabular osteoplasty and labral repair, which may also avoid the complications of femoral-side decompression.
“Good to excellent patient-reported outcomes and satisfaction scores were noted with significantly higher scores in patients with minimal arthritic change. Patient age and preoperative alpha-angle had less effect on postoperative outcomes,” researchers wrote in their study.
Using prospectively collected registry information, they identified 86 patients who underwent arthroscopic acetabular osteoplasty and labral repair for femoroacetabular impingement and labral tears. Investigators recorded the preoperative alpha-angle, degree pf radiographic degenerative changes and crossover sign presence. The modified Harris Hip score (mHHS), International Hip Outcome Tool-12 (iHOT-12), Hip Outcome Score-Sport Specific Subscale (HOS-SSS) and patient satisfaction score were used to assess clinical outcomes at minimum 2-year follow-up.
Results showed significantly higher mHHS, HOS-SSS and iHOT-12 scores for patients with a Tonnis grades 0 or 1 changes compared with patients with Tonnis grade 2 changes. Patients with Tonnis grades 0 or 1 had a mHHS of 83.5 vs. 75.1 for the Tonnis grade 2 group. In addition, HOSS-SSS for the groups were 81.3 and 59.9, respectively. Investigators noted patient satisfaction scores were not different between the groups, with the Tonnis grade 0 and 1 group having a score of 8 vs. 7.2 for the Tonnis grade 2 group.
The unilateral hip patient outcome scores and the bilateral hip patient outcome scores were not significantly different. Outcome scores were not associated with the age of the patient or the preoperative alpha-angles. No cases of revision or progression to arthroplasty were seen. – by Monica Jaramillo
