High cholesterol levels, no statins linked with increased failure rate after arthroscopic rotator cuff repair

SAN DIEGO — Patients with moderate or high cholesterol levels and who were not prescribed statins had a greater risk for failure after arthroscopic rotator cuff repair, according to results presented here.

“The present study found a significant association between moderate and high perioperative total cholesterol and LDL levels in the rate of revision arthroscopic rotator cuff surgery after primary arthroscopic rotator cuff repair,” Brian C. Werner, MD, said at the American Academy of Orthopaedic Surgeons Annual Meeting.

Werner and colleagues stratified patients who underwent arthroscopic rotator cuff repair and had perioperative serum total cholesterol, LDL or triglyceride levels into low, moderate or high levels based on guidelines by the American Heart Association.

“Within each group, the patients were then subgrouped according to documented statin prescriptions given within 6 months of surgery,” Werner said. “Our main outcome measure was revision rotator cuff surgery.”

Results showed significantly higher revision rates among patients with moderate or high total cholesterol levels vs. patients with low cholesterol levels. However, Werner noted the difference was due to patients not prescribed statins.

“Patients who had moderate or high total cholesterol levels, but were taking statins at the time of surgery, their rates of revision were not significantly different from normal controls,” he said.

Werner added significantly higher revision rates were also found among patients with moderate or high LDL levels, with the difference due to patients not prescribed statins.

“Finally, we broke the patients down based on their triglyceride levels,” Werner said. “You can see there was no significant associated between triglyceride levels or revision surgery after arthroscopic rotator cuff repair.” – by Casey Tingle
