Smoking and revision discectomy cited as predictors for fusion after microdiscectomy
BOSTON — A presenter here said patients overall were ultimately unlikely to need fusion at the index level following microdiscectomy, even with identified predictive factors that included smoking status and history of revision microdiscectomy.
“What we found was that smoking status, as well as what you might not find surprising is that revision discectomy or having a revision discectomy, was also significantly predictive for those who will eventually needed fusion,” Samuel C. Overley, MD, said at the North American Spine Society Annual Meeting.
Overley and colleagues performed a retrospective review of 174 patients who underwent a single-level microdiscectomy with either the mini-open approach or with a minimally invasive surgery (MIS) dilator tube. Investigators used multivariate regression analysis to determine predictive factors for patients who would need a subsequent index-level fusion.
“We also did the chi-squared analysis comparing revision discectomy in patients who went on to eventually require fusion and not surprising, again this was a rather significant finding, a correlation between these patients,” Overely said.
Findings showed regarding patient factors, BMI and length of follow-up were the only differences found between the open group and MIS group. Overley noted both groups had similar revision discectomy rates. Patients who underwent the mini-open approach and those who underwent the MIS approach were not statistically different regarding the rate of patients who needed an index-level fusion (5.5%). Revision discectomy and fusion occurred within the first 18 months and 36 month, respectively. The Kaplan-Meier curves showed the survivorship of revision discectomy stabilized in 18 months and 36 months for patients who underwent fusion. – by Monica Jaramillo
